Products: Select 3M™ DBI-SALA® Ultra-Lok™ Self-Retracting Lifelines

3M is issuing this “Stop Use/Remediate” notice for select 3M DBI-SALA® Ultra-LokTM Self-Retracting Lifelines

As part of 3M’s ongoing commitment to delivering high quality products, we are notifying our customers of the following information related to select 3M™ DBI-SALA® Ultra-Lok™ Self-Retracting Lifelines. 3M Fall Protection has identified a potential supplied raw material issue in select 3M™ DBI-SALA® Ultra-Lok™ Self-Retracting Lifelines produced between November 17, 2023, and June 14, 2024. 3M has determined that in the event of a fall and under certain conditions, these devices may not lock up properly or prevent retrieval models from going into retrieval mode which could expose the worker to serious injury or death. There have been no reports of accidents or injuries associated with this issue.

Due to this, we are sending out this Stop Use and Remediate notice, so the select 3M™ DBI-SALA® Ultra-Lok™ Self-Retracting Lifelines are removed from service immediately. Failure to read and follow all instructions associated with this Notice may result in serious injury or death of a user. Instructions for appropriate next steps are below, see the appendix for a complete list of models impacted by this stop use notice.

 

End Users

Please follow the steps listed below:

Step 1:
Locate the model number, manufacture date, and lot number on the device information label (see Photo 1). If your device model number and manufacture date or lot number is listed in the appendix, proceed to step 2. If your device model number and manufacture date or lot number is not listed in the appendix, it is not impacted by this notice.

Step 2:
Please submit a request to have your device(s) repaired by our 3M’s Service Center by completing the SRL Repair Request form in the QR code to the right. Our 3M Service Center will repair your device (s). The repair and shipping will be at 3M’s expense. Once you submit your request, 3M’s Block Repair Customer Service department will email your repair authorization number and return shipping instructions.

 

Distributors

Upon receipt of this Notice, please contact your 3M Fall Specialist for a listing of the impacted products sold to you. If you have any of the impacted 3M™ DBI-SALA® Ultra-Lok™ Self-Retracting Lifelines in stock, please submit a distributor stock repair request by completing the Stock Repair Request form in the QR code to the right. Our 3M Service Center will repair and return your stock. The repair and shipping will be at 3M’s expense. Once you submit your request, our Block Repair Customer Service department will email the stock repair authorization number and return shipping instructions. Please immediately forward this Notice to any of your customers who have purchased impacted products from you and provide any assistance requested by your customers to complete the process.
